The Joint Commission's Disease-Specific Care Certification for Diabetes is designed to evaluate clinical programs across the continuum of care and may be appropriate for large systems and hospitals. Organizations may seek certification for diabetes using the three components of the program: Standards, Clinical Practice Guidelines, and Performance Measurement.
